Family Health Teams (FHT) are primary health care organizations that include a team of health care providers (e.g., nurses and other professionals) that work collaboratively with affiliated/associated physicians and other community providers to ensure that people receive the care they need in their communities. Each team has programs and services designed based on local health and community needs.
Ear Falls Community Health Centre/Family Health Team is dedicated to providing high-quality, patient-centered care to the residents of Ear Falls. Our experienced health care professionals, consisting of one doctor and three nurses, collaborate to deliver innovative and compassionate healthcare services to promote overall health and well-being.

Our healthcare team provides a specialized Well-Women Program through preventative care and comprehensive shared care for women's prenatal care.
We offer compassionate mental health services to support your emotional well-being with mental health first aid and resources as needed.
Our team is dedicated to providing expert paediatric care for the youngest members of our community with our Well-Baby Program and routine Childhood Immunizations.

More Information and FAQs
1. What if I miss a scheduled appointment? If you are not able to attend your scheduled appointment please call us as soon as possible so we can make appointments available to those that need them as Dr. Khan's time is very valuable. If you DO NOT show up for your appointment you will be charged $30 for each missed appointment. You will have to pay the fee in full via etransfer (adelorme@nwocs.ca) or cash at the reception desk BEFORE another appointment is scheduled. Thank you for understanding as we try to do our best to serve our Ear Falls community members.
2. Does the Ear Falls Community Health Centre have walk-ins? NO. If you are experiencing an emergency or urgent matter please go to your nearest emergency department or urgent care clinic in Dryden or Red Lake. In order to be seen at the Ear Falls CHC you must be a rostered patient. To make an appointment please call 807-222-3728 and a nurse will call you back for more information as soon as their schedule allows.
3. Can I book an employer or MTO medical with the doctor? In order to be seen at the Ear Falls CHC you must be a rostered patient. To make an appointment please call 807-222-3728 and the receptionist will book your 40 minute medical appointment in the next available spot with the nurse and the doctor. Please be aware there are times we are booking 3-4 weeks in advance.
4. What if I am not a rostered EFCHC patient? First you must register with HealthCare Connect at: www.ontario.ca/healthcareconnect or call 811 24 hours a day, 7 days a week. After registering, if you live in Ear Falls, you may fill out a New Patient Intake Form (found in the FORMS section). Once completed and submitted you will be called to schedule an In-person Intake appointment with the nurse.
5. What if I do NOT live in Ear Falls or I need to be seen sooner? You may visit the nearest Emergency Department or Urgent Care Clinic (in Red Lake or Dryden) or Virtual Nurse Practitioners are available 1pm -9pm daily 7 days a week at www.nwvirtualcare.ca or 1-888-684-1999. If you do not have a family doctor please register for Health Care Connect at www.ontario.ca/healthcareconnect or call 811 24 hours a day, 7 days a week.
6. What is Tali AI? Tali is a digital assistant that helps your health care provider focus on you instead of the computer. It creates a written record of the conversation and generates a summary for your medical chart. Every note it creates is reviewed to ensure it is accurate, which helps your health care provider ensure you get the best care possible. 7. Do you offer Telehealth? This service is available in our clinic. Patients may contact their specialist's office to inquire to have Telehealth appointments set up as we do not initiate or start the calls. Telehealth is the use of digital information and communication technologies to access health care services remotely and manage your health care. Telehealth: Makes health care easier to get for people who live in communities that are remote. Offers access to medical specialists. Offers advice for self-management of health care.
8. How can I get my paperwork completed by the doctor? Patients rostered with Dr.Khan can make an appointment with him to have their paperwork completed. The paperwork must be dropped off and a copy given to the receptionist for the patient's chart upon booking the appointment. Paperwork fees can be paid in Cash or via Etransfer to adelorme@nwocs.ca
9. How can I get my prescription refilled? Prescription refills can be called into the Ear Falls Pharmacy at 807-222-2038. The pharmacist will send a fax to Dr.Khan who can then respond. If the pharmacy asks that you call the clinic for your refill it is likely that you need a NEW prescription and not just a refill. You will need to speak with a nurse who can send an electronic message to Dr. Khan who will respond appropriately. Please be advised this can take a few days so please plan ahead. Thank you.
10. Do I need an Ontario Health Card? Yes! We require an up-to-date OHIP card number with version code in order to properly bill and track services provided to you under OHIP (Ontario Health Insurance Plan). Failure to provide a valid version code may restrict your ability to receive imaging, laboratory testing and other OHIP funded services. If you are an existing, registered patient, please take the opportunity to provide us with your updated health card information by filling out the "Your Health Matters to Us" form below.
11. Can my test results be e-mailed? Test results can be provided upon request but not without risk via e-mail. Please be advised that e-mails should be confirmed along with consent prior to sending any personal health information and be aware that standard email is not secure, and information could be intercepted or misdirected.
